How to make the bug occur: Run Campbell's Last Breath (Nurse Very-Rare addon)
How often does it occur? Sometimes, but It is not consistent.
As far as I've seen, what causes the nurse to sink into the map is collision, if the second instant blink landing zone is blocked by something, SOMETIMES, the nurse will dive into the map and reappear. In order to showcase this occurance here's a short clip from the brazilian fog whisperer SouzaSoul, a friend of mine.
My theory of what's going on: Notice that he had plenty of objects to go through, and since the blink didn't allowed him to go further, he was sent back, but the place he was sent back was had a hitbox and made him sink into the map.
