Successful 4% Unhooking Put Me Into Hook Phase 2

I unhook myself in the last few seconds before hitting stage 2 of the hook phase, but the game decided to count my unhook as stage 2 of the hook. Next time I got hooked I died, so it wasn't a UI bug.
My guess to reproduce this is just to unhook yourself right before you hit phase 2.

step two: play survivor
step three: got hooked by killer
step four: self unhook
step five: notinced i was on death hook, second phase
step six: gets tunneled by killer
step seven: gets hooked
step eight: death.
