Hysteria not always activating when survivors become injured

Hysteria - Whenever a healthy Survivor becomes injured by any means, all injured Survivors suffer from the Oblivious status effect for 20/25/30 seconds. Hysteria can only trigger once every 30 seconds.

As the Wraith, I uncloaked and successfully interrupted a survivor repairing a generator. Grabbed survivors immediately enter the injured state, but this did not activate Hysteria even though the perk says "by any means."
