Tile on sanctum or wrath missing window

The tile on sanctum of wraith that's supposed to be a jungle gym spawned with two open sections instead of 1 window section and 1 pallet section (the second section didn't have a pallet)
Platform pc (not that I think it matters in this case)
Portion of a tile spawned incorrectly was supposed to be a window but didn't spawn the window so there is just an opening with no pallet and now window on that half of the tile
Steps to reproduce I guess you could try booting the map a few times and check if this tile is glitched out mapwide or in this spot specifically
How often does this occur unknown
