Losing items after escaping

Description: Several times i've ended up losing a Commodious Toolbox and other items even though I've successfully escaped trials. The killer didn't have franklins demise and I obviously didn't die or drop the item.
Unable to reproduce at the moment.
How often? This has happened several times over the last few months
