PS4 - Huntress Hatchets does not injure PC[?] survivor

Was playing Huntress against a lobby of two PS4 survivors, and two non PS4 survivors [PC?]. Was able to injure three survivors with hatchets throughout the match, except the fourth; a Jeff. Did not see him the entire match, until I catch him on final Gen. I throw two hatchets at him. Both hit him. Squish. Squish. But he remains unphased/uninjured, keeps doing Gen. I literally touched him with my body., then he suddenly gets injured and disappears.
Not sure if this was a bug or a hack.

It happened again. I noticed both times two survivors were on Entangled Gens.
