Getting stuck on a window when fast vaulting,

I was playing dwight with quick and quiet, head on, no one left behind, and I was playing on xbox one,

Steps to reproduce

1. Fast vault any window

This happend to me on azarovz auto haven wrecker maps. At a T&L loop

    Playing on PS4 i am getting stuck every time i am vaulting when i am chased by the killer, if the killer is not behind me i can vault normally. It is unplayable on ps4 right now. Then i am suddenly teleported on a hook and dead.
