I think Jeff is a bit underrated
I think his perks are all pretty good in principle, but something is holding them back from being very good. For example "Aftercare", the principle of this perk that you can see other surivors with whom you have previously interacted, is a very good approach and has the potential to be very helpful. But this effect of resetting everything when you're hooked is just stupid and destroys the whole perk.
I understand that you might not want to make the perk too strong and therefore weaken it a bit. But I think this has been exaggerated and this effect has to be removed or nerfed. If you're on the hook, for example, you'll be blind for 60/50/40 seconds or something.
"Distortion" is a good perk on its own, too. It allows counterplay against information perks like "I'm all ears" or "barbecue and chili". But maybe 3 stacks are not enough. Maybe the perk needs a mechanic to charge the stacks. You start with a stack and with every unhook or every healing you get a stack on the perk up to a maximum of 3. These stacks can then be recharged again and again and can also be easily combined with "Aftercare".
The final perk, "Breakdown", may not be particularly powerful, but it can come in handy at the right moment. I don't think he needs an urgent change, even if you could definitely work on the perk. But not all perks can be super strong.
I think with these changes Jeff could become a very good survivor. I actually like him and I think he has a lot of potential with his three perks. Maybe we'll see more Jeffs in the game in the future than we do now.

Aftercare- there's a reason that it works the way it does.... Empathy does the same thing but in a range
Distortion- I can understand why they picked it to only have three charges... and also understand why it needs more but with Shadowstep in the game now it seems unlikely
Breakdown- like you said it has it's uses... not much can be done to this perk though

I would prefer to see Distortion work on a 120 second cooldown, with unlimited use.
That said, I genuinely believe its a good perk as it is, and the only reason I don't use it is because I trust myself in chase more than I trust my teammates. If the killer can't find me, they can't chase me, and historically that leads to me never escaping the trial.
