Not able to make a flashbang

Hi, I was playing a game today as Claudette having:
- Head on
- Distraction
- Flashbang
- Quick and quiet
as perks.
I created 2 flashbangs but when trying to create my 3rd one I did not get the message prompt to create it.
This has happened in only 2 of my games in about 300-400 games so it doesn't happen very often.
