A List Of Everything That Causes Hitching on Console. (Freezing)
Please note that these hitching sources are not 100%; they occur often but not every time. The most consistent sources of hitching are definitely the hooks and completing generators.
Hitching Causes Include:

Hooking a survivor (Caging does not seem to apply)
Unhooking a survivor (Uncaging does not seem to apply):
Completing a generator
Taking chance to escape and succeeding
Vaulting a window within the render distance on console; but outside of chase. (or it's just Lithe) I will try to get more data.

Finishing a Tombstone Mori
Doctor:
Iridescent Queen activation
Nightmare:
Dropping an illusory pallet

Removing Victor from a survivors back (Survivor crush action; this does not apply to Charlotte recalling Victor)

Injuring a survivor with Showstopper
Downing a survivor with Showstopper (Inconsistent)
Nemesis:
Infecting a Survivor (Turning them blue. Consecutive hits with Tentacle Strike do not cause hitching)
Using a vaccine
Leveling up Tentacle Strike to level 2
Leveling up Tentacle Strike to level 3
Cenobite:
Getting hit by Possessed Chain
Artist:
Getting hit by a Dire Crow (Causing a damaging state)
