Killer not able to pick up/mori last survivor

Step 1: Picked Nemesis running Pop Goes the Weasel, Corrupt Intervention, I'm all ears, Eruption (Add-Ons: S.T.A.R.S Field Combat Manual and Visitors Wristband / Offering: Cypress Memento Mori)
Step 2: Queue up and suvivor offerings were Midwich and Haddonfield
Step 3: Play game til final survivor is downed (3 survivors were hooked no mori or bleed out) (no hatch closed or gens finished)
Step 4: Wasn't able to pick up or mori last survivor
Added note: They were not losing blood on the UI and I wasn't sure if they were hacking but they died and the bar disappeared. I don't believe they were hacking but it was weird.
