Dying Instantly on Second Phase

Platform: PC

I was just chilling on the hook, then suddenly I got sacrificed instantly on hook once I reached phase 2.

I don't even know what happened or what caused it, maybe because the killer is Ghost Face?

It only happened once so far but it was a weird bug.

Edit: Oh yeah my game also crashed.

    it's a visual bug, the hud sometimes doesn't update the hook counter when a survivor reaches second hook stage

    No I've seen this before to.

    Someone was on a hook for the first time and the second they reached phase 2 they just straight up died. I've only seen it once though.
