Your Post-Match Survey is deeply flawed

Almost every question on this survey makes no sense given the context of the game.

"Do I feel like I won this match" and "I enjoyed this match" are the only two question that make sense

"Did you play this match with a more casual mindset or a more competitive mindset" More often than not your mindset will change throughout the match, or at the very least until you know which killer it is.

"I feel like this match was fair" and "I feel like this match was frustrating" go hand in hand. There is no reason to have both of these question. If you don't feel the match was fair then the match was frustrating to play.

"How would you rate the skill level of the Killer compared to your skill level" is a useless question without giving the player any ability to provide context. Something along the lines of "what is your decision based on?" and allow the player to comment on it.

"How would you rate the skill level of other survivors compared to your skill level" has the same issues as above with an addition caveat, if you have 1 bad teammate but the other two are decent what do you put? If you have 1 god tier teammate but the other are decent what do you put? If one of your teammates DCd what do you put? Without some way to judge each individual teammate this question is useless.

Then there is the problem of there just simply isn't enough questions for a survey like this. There really should be questions for "did someone Disconnect or otherwise play in a way that is unintended" but there isn't and that is really crucial to the experience in a given match. If someone DC's at the start of the match its obviously "not going to feel fair". The same can be said if someone kills themselves on a hook, or is AFK for the first 2 minutes of a match, or if some exploit was used or if the killer decided to just let everyone live for some reason.

Simply not having a small comment section is a huge mistake. It would let players explain why they chose the things they did.

This is all not to mention how absolutely horrendous the implementation of the survey is. You have to click a link in game, so that you can click a link on a web browser to get to the survey. Then once you do that you have to go back to your game screen, location your Cloud ID, and manual copy over each individual character (32 characters btw) into the survey. There is no reason for this to not be copy/paste able (personally I copy / pasted into a notepad but it shouldn't have to be that way). Then you have to play a match and stay at the end game screen to look at your exact number of points and manually copy that over as well. Once you do that fill out the survey, and then... open up a new one?? Why does it not just automatically lead you to a fresh survey?

And then ontop of all that you want players to do that for 7 consecutive matches. This is probably the biggest flaw imo. There are a few situations where the player just has to restart the survey. If they DC from a match -> Restart. If someone DC's during the load screen -> Restart. If they forget to look at their blood points at the end game screen -> Restart. If they accidentally clicked on the Archive thing that pops up rite over the chat back because they wanted to say something in the end-game chat -> Restart. (that last one is actually something I have hated for so long, why does the "claim rewards button" pop up directly ontop of the chat box?).

You also choose to implement this survey during the same week that you are having a much higher rate for very specific maps to show up which may or may not have an impact on how players will perceive a given match. There are just so many flaws with it.

I am still going to fill it out, but that is only because I really truly want this game to climb its way out of the gutter it fell into and become a good game again. But BHVR, you are doing the absolute minimum to make that happen.
