Hook counter visual bug

This person apparently died at the same time as they were saved, possibly a lag issue but my connection is fine. After a short delay their portrait went from being sacrificed to injured, netting me a hook 'stack' in the process
at the end of the game I had '13' hooks, though it looked like 12 still

Is it, by chance, something like this?
(Nea supposedly died on hook... right as she got saved. Not sure if she was broken or AFK after that.)
