How does the clowns pink add on work

I've heard people mention it in a party chat before when I was going for his adept trophy and seen it used but do you need to hit them with the bottle or just the cloud

He has two pinks, one is not very useful (Tattoo's Middle Finger or some such), but the other is sweet: Redhead's Pinky Finger. You have to actually hit a survivor with the bottle, not just the cloud, but that's not extremely hard to do. Then they'll be Exposed: one hit and they're down.

Exposed effect only lasts as long as the Intoxiaction effect, so using it together with Ether 15% will help you a lot by extending that few seconds.

But my favourite thing to do is use it with an area expansion addon. Then, I just aim at the survivor's side at the beginning, so they won't realize I'm using Redhead's Pinky. And then, in an opportune moment, I start aiming directly at them. Good for quickly getting rid of a pesky survivor who's rushing to unhook someone.

Mostly though, you must be careful around loops: there's no way to get them while they're still Exposed if they have a window or pallet handy. Don't give away your filthy little secret until you're sure you can hit them
