Daily Ritual - Escape with a new item bugged

PC Steam - My wife had a daily ritual to escape with a new item (not character specific). She brought in a commodious toolbox and traded it for a brown one she found in a chest she opened. After escaping through the exit gates, the quest was not complete.
She redid it bringing in no item and escaped through the exit gates with a yellow toolbox she found in a chest she opened. It completed that time.
