PC - Sole Survivor bugged

Played a game as Ada on Fractured Cowshed. Perks: Low Profile/Sole Survivor/Spine Chill/Wake Up!.
Three Survivors escaped before I did. After a minute or so, I received a Sole Survivor token. A few seconds later, I received a second one. I'm not sure why I didn't receive a third, based on what happened, nor why I received any, at all, for that matter. I've not been able to reproduce the circumstances during which this occurred, but if I am able to do so at some point in time, I'll update this.
