Dredge not getting adept

This is the second time i’ve attempted to get adept dredge and it isn’t working, I play on pc (steam) and I’m using dredge with only his three perks, malthinkers skull and worry stone addons, and rpd badge, i hooked each survivor 3 times kicked a bunch of gems and got 4 irisescent grade badges and a double pip, i also got “merciless killer” after the match, this has now happened twice and i’m wondering why am i not getting the achievement?
