Help my friend fell through the floor

I have no idea what happened here, but They were on the killer's shoulder and another player was body blocking killer so the killer dropped my friend right as they were about to get off but it was too close to a wall and they fell INSIDE the ground. We could not touch them. We could not heal. We could not have them be picked up by killer. What happened here?
