Getting Meciless Killer is not enought, sometimes it does not count on your progression
I am usually using Fearmonger, Bitter Murmur and Hex: Thrill of the Hunt, and even with 4 kills (or 3 kills 1 dc) at the endgame screen "Merciless" appears, but at the achievments it does not count.
Itt occoured like 5 times already.
