Adept achievement bug on Wraith

Using the 3 perks Bloodhound, Predator, and Shadowborn and getting a 4k did not give the adept achievement. This is the second time this has happened specifically with Wraith, I have recently gotten the achievements for the Executioner, Deathslinger, Ghostface, Clown, pig, and Trickster without problem but its happened twice with Wraith.
I've attached a video showing both the double pip and the 4k. The only off thing I can notice is that there was a DC. I do not remember if my prior attempt also had a DC.
