Sloppy Butcher status effects aren't applying

Description: Sloppy butcher, or perhaps mangled and hemorrhage in general, doesn't apply the status effects when survivors are actively using items.
Step 1 : Play as killer and equip sloppy butcher
Step 2 : Play as survivor and equip an item (tested with Flashlights and the pink Map)
Step 3 : Survivor uses the item while the killer hits the survivor (e.g. survivor channels a pink map while getting hit)
Step 4 : Notice that the status effects from sloppy butcher don't apply to the survivor
How often does this occur - Every time that I've tested it.
