It's definitely not something I'm doing wrong, I'm using ONLY his 3 perks, I've got a merciless victory 3 times now, 9 hooks in 2 of the games just to be safe, nobody dying to bleedout/endgame timer/anything like that, I've made sure I have the right perks equipped, but still it's just not giving me the trophy, I don't understand why, is it just bugged or is it only for me?

It's all Adepts, unfortunately. They do seem to be bugged. Check the main thread about it further down.
