I didn't get my adept achievement for the twins

I didn't get my adept achievement
"Merciless killer" appeared in my screen for 3 Games in nearly a row and the Achievement didn't show up.
Also i lost addons like "SILENCING Cloth" although i used "BLACK WARD" as Offering.
I don't care about the addons, buts seems to be a bug also.
More annoying is that the Achievement is not reachable at time even you make "Merciless killer" with or without 2 pips and 4k.
