[PC] Adrenaline did not trigger
Issue:
As you can see, there is no reason for Adrenaline not to trigger in this case: The Knight did not hit me (Endurance still up), no Terminus or similar which causes a Broken Status.
My best guess is, that the last gens were completed nearly at the same time that the game did not properly registrated it. Maybe similar to the old bug when the game would not register that the 1st gen was completed too soon.

That's really strange.
My guess is that it has something to do with OTR. This will be deactivated when the last generator has been finalized. At the same moment that the unhook happened, it was still active very shortly before. Maybe adrenaline was deactivated for some reason too, I don't know the exact code in the background, but it also has to do with an extra hit/health state (OTR / adrenaline).

I really think it's the timing.
When you were unhooked there is a very brief moment where OTR was still triggering shortly after that the last generator went on while you were still in the process of unhooking -> OTR was disabled and adrenalin was not triggered.
I think it was just a couple of circumstances with unfortunate timing that will be hard for the Dev-Team to fix. As you said yourself, this has only happened once so far.

OTR does not change my status. I had OTR active many times (with endurance still intact) and it did not make problems. Usually Adre should trigger independent of your status. Maybe it was because of the i-frames during the unhook.
