Exit gates needs better placement
Sometimes the gates are placed very close to each other and the killer can just stand in the middle and have a clear view of them, what's the purpose of that? Some killers have a better mobility and you just don't have a chance to open the gates.

Sometimes the exit gates spawn on the opposite sides of the map and the killer has absolutely no chance of patrolling them.
RNG giveth and RNG taketh away.

Win the match and run out with the team? – Nah, not for me
OK, maybe try your luck and try the hatch? – No, definitely not enough, need something else
Then what do you want? – I want to have a third GUARANTEED chance to escape from the match
What's next? Complains about killer not standing still?

Changes I'd like to exit gates
1. Lengthen gate opening time
2. Gates slowly lose progress if you stop opening them to prevent 99%
3. Gates spawn at farthest possible distance from one another.

It would be interesting if
a, gates spawn further away from each other to prevent possibility of direct line of sight between them
b, there was guaranteed some kind of visual obstacle, so that killer does not circumvent it by going on a hill/top floor of building/etc and see both sides.
BUT AT THE SAME TIME: Killer looses terror radius and becomes undetectable during EGC when there's just single survivor left.
This way the game changes from just killer running from gate to gate and trying to see both gates at shortest possible interval into kind-of hide & seek game. This would allow for killer to a, patrol as he does now (but with lesser chance of success) b, hide next to one gate c, try to track survivor at some common crossroad between gates. b, and c, options are not really possible (c, is sometimes possible on very few maps like azarov's or maybe RPD) currently and a, is almost entirely dependent on map RNG instead of player's agency.

You just have to accept the RNG in Exit gates.
What would you do in Midwitch? Those are fixed and it's basically impossible to escape on those gates because they are so close to each other, any killer can patrol those gates very easly.

So:
- Your teammates died, you lost your genuine escape chance
- You couldn't find hatch, you lost your second escape chance
- You want a free gate escape as well? What?
If anything gates should spawn closer to together. If survivors finish gens, it doesnt matter how close they are, those gates will get 99'd and opened up.
Its not fair to the Killer that he has to perform well, then get the RNG to find hatch, then be forced to close hatch since there's nothing preventing the Survivor from camping a gate, then hope that he has to get the RNG for gates to spawn close too.
Sometimes a gate is SO far from the hatch that you cant even walk there before the gate is opened.

I feel like gates should have fixed spawns much like they do in indoor maps

As person who plays killer quite a bit I have to disagree with these changes.
Mainly slowly regressing gates to prevent survivors 99%ing the gates. Also having the gates spawn at the further possible points would only make sense if map sizes were standardized across the board because if you don't do that then there are maps where most of the killers literally can't do last minute defense due to not being able to traverse the points.
Lengthening the gate opening time would probably be fine, but honestly the current time it takes is fine and can be augmented with multiple perks which in one of few times that perks to solve an issue makes sense.

i had a game last night as killer where the exit gates where in direct line of sight from each other. I hardly had to go anywhere. Definition not balanced.
RNG could be better.
