Rift Challenges Not working

Xbox One - Sometimes when doing rift challenges I will complete them or progress them during games however when it comes to the end of the match, either escaped or sacrificed as survivor / killing all survs or having them escape as killer, the challenge does not update, I then cannot access the rift through the character select screen and will have to exit to the main menu to do so. this also happens with the xp rift reward fragments, as they do not get rewarded until I have closed them game for a few hours and then returned. this occurs maybe once every day to once every 2 days and it happens for a few hours at a time.
