PC - Onryo tape not removed on hit while depositing tape

Playing as James Sunderland on Sanctum of Wrath. I had only 3 stacks of condemned and retrieved a tape from a tv. I went to another tv to deposit the tape, and the onryo arrived as I was putting it into the tv. I was hit while placing the tape and received two stacks of condemned, but the tape was not destroyed or returned. I was immediately hit again and received another two stacks of condemned, making me instantly killable.
It would seem there is some bug preventing the tape from being properly destroyed if you are hit while in the process of returning the tape.
