PC - Yet another Pinhead break with lament configuration
Step 1 : Loaded into the game as Pinhead
Step 2 : Survivor picked up lament configuration
Step 3 : Survivor holding the lament configuration was downed. Pinhead solves the box, but the chain hunt was restarted as opposed to instantly going into the actual chain hunt.
Step 4 : Yet another survivor picks up lament configuration
Step 5: Survivor holding the lament configuration is downed. This time when Pinhead solved the box, on the HUD it showed that chain hunt had started but it instead completely broke and no chains spawned for the rest of the match.
Pinhead was the killer.
Perks: Rapid Brutality tier 2, Save the Best for Last tier 3, Brutal Strength tier 3, Jolt tier 3.

I know this is not going to resolve the issue but holding forward when downing a survivor with the Lament Configuration prevents this bug from happening.
Try not to stand still with the box. I believe the game thinks you are standing over the box thus restarting chain hunt instead of starting it. I hope this helps for the meantime.
