I have completed two games and never got the prompt to install 'blast mine'. The maps were Blood Lodge and Crow' Eye.

To activate Blast Mine (or Wiretap), you can no longer do so while actively repairing a generator. You must momentarily stop, and while standing by the generator, will get the UI prompt to install the perk.
