Pig - RBT addon issues

Three of Pig's addons which apply status effects when installing a RBT (Face Mask, Utility Blades, and Slow-Release Toxin) have a couple of problems.
First, the addons do not work correctly if the same survivor gets more than one trap. The addons will work on the first trap placed on a survivor, but if the survivor removes that trap and then gets downed again and receives another trap, all of these addons will have no effect the 2nd time.
Second, these three addons don't display their names on the HUD properly with the status effect notification.
Worth noting, Rusty Attachments (the Mangled addon) does work properly, it is only the other 3 that are bugged.
