Doctor Madness III - display issue or Bug?

Today I playewd against doctor. After he used his Static Blast, I screamed and the HUD showed I am Madness III, but the icon to the left showed I am Madness II

I was working on a Gen and directly getting back to working at it. Unfortunately I don't remember my Madness Level before that event. In addition the "small dots" moved slower than normal Madness III HUD animation.

As of now it only occured once to me.
