Unable to unhook on second stage

this has happened to me 3 times now since the mid chapter update where I get hooked for the first time in a trial but the game thinks I’m second stage and when the bar depletes and I actually go into second stage the games thinks I am just dead and I become unhook-able for my other teammates. They don’t get any prompt to unhook me.
