White Ward offering is not working on PS5

The white Ward offering is being consumed but so are the items and the addons (which it is supposed to keep) when you die in the game
Play a game with a toolbox and 2 addons that give more charges and the offering equipped
It occurs every game

Did you die still holding the item or did you put the item down, or did the killer have Franklins Demise etc?

I still had the item in hand. I believe the issue could be related to the way the item drops on the floor after you are sacrificed on the hook.

DC'ing from the match could also be the reason for losing the item + addons.

Losing the White Ward through DCing is intended.
