Outta Nowhere achievement is bugged on PS5

The Outta Nowhere achievement, to hit 40 survivors while undetectable, is stuck at 30/40 completed on the PS5 edition. I’ve tried using Trail of Torment and hitting survivors, so it should be increasing, but it seems to have stop counting.
It’s also worth noting that the Skilled Huntress achievement also stopped counting at 50/100, even when it definitely should be counting. Is this an issue with the PS5 achievement system?
