Ada disconnected and the whole game she was shown as on hook, but you couldn't see her on the map. Her body was physically there, you could bump into it.
The bot being so bugged out meant that the hatches didn't spawn for the last 2 survivors and that the killers couldn't leave the game. Eventually the hatches spawned, got closed and timer ended, but she was still there. Only a few minutes later did the game end.
