Clean Break Not Working

SUMMARY OF THE ISSUE: Played against Twins on Eyrie of Crows. Was unable to use Clean Break at all. The prompt would display and the noise for activation would play but the perk would not trigger. This happened consistently through the match and I was never able to get value from the perk.
PLATFORM: PC
STEP BY STEP INSTRUCTIONS ON HOW THE ISSUE OCCURED: Play against Twins on Eyrie of Crows. Try to use perk. Perk doesn't work. Sound plays from perk but no perk activate.
