Chapter: Veil of Pestilence
“Where sickness spreads, courage must endure”
In the shadow of the Black Death, a masked doctor wandered the plague-ridden streets of Firenze, spreading both fear and fascination. Known only as The Plague Doctor, Niccolo Vieri vanished into the mass graves of the afflicted, leaving only whispers of his “treatments.” Now, drawn by the Entity, he roams a realm where death and disease intertwine, seeking fresh subjects for his morbid experiments.
Opposing him is Alessandra Rossi, a healer hardened by tragedy. She witnessed the doctor’s cruelty firsthand and refuses to let him claim any more lives. Her courage fuels her allies, and her determination turns the tide against the creeping sickness.
Face the spread of disease and the shadow of death in the Veil of Pestilence, where survival demands both teamwork and resilience.
Dr. Niccolo Vieri – The Plague DoctorBorn in 1318 in Firenze, Italy, Niccolo Vieri was the scion of a family of learned medicinal scholars. When the Black Death swept across Europe in 1347, Vieri became one of the first to volunteer as a physician, and remained a well known Doctor, becoming moderately famous for his miraculous remedies. Clad in the beaked mask, waxed coat, and wide-brimmed hat that would become his chilling signature, he ventured into the homes of the dying, his hands steady even as death clawed at him from every side.
Yet repeated exposure over the years to the plague left its mark. Vieri fell ill, but rather than succumb, he found in the sickness a grotesque clarity. Life, death, flesh, and decay revealed themselves in sharp relief. He began experiments that twisted medicine into monstrosity, at times even on himself, learning to command the plague itself, sustaining himself in a state of perpetual infection, thus becoming the very sickness he once cured. Though he was able to keep most of his body in this state, Vieri would experience pain and agony with every step, every breath. Death became a companion, the sickness a crucible — and through it, Vieri sought to achieve true purification.
To him, the plague was not calamity — it was revelation. The screams of the dying became progress, their decay a lesson in the fragility of flesh. The “cured” were only delayed; the purified were those who had embraced suffering. Some unfortunate patients would writhe as the plague consumed them, only for Vieri to reverse its effects, then strike them with the disease anew. Their skin would bloom with putrid green spores, a grotesque signature of his experimentations. None survived.
Whispers of his deeds spread through Firenze: the doctor who courted death, who wandered plague pits at night, muttering calculations of contagion. His colleagues recoiled in horror upon seeing his victims; his family repudiated him. Soldiers eventually came to seize him, but Vieri and his experiments vanished into the rolling Tuscan hills.
They said that after months of searching, they finally cornered him near a field of mass graves, a ghostly gray fog tinged with sickly green rose around him. Vieri’s voice, calm and unwavering, declared that perfection was near. Then, as if swallowed by the plague itself, he stepped into the mist — and vanished. No trace of him, no sign of the glowing fog, remained.
The Entity welcomed him. In its realm lay a world of endless suffering and decay, Vieri found a laboratory unbound by mortality, unshackled by weak morality. Here, the sick never truly die. Here, the living are prey, the dying subjects of his experiments. Every scream furthers his understanding, every suffering body another brushstroke in his grotesque masterpiece. Behind the luminous green glass of his mask, Niccolo Vieri watches, unblinking, awaiting the next soul to be purified.
Born in 1329 in Firenze, Alessandra Rossi was the daughter of a tailor and a midwife. From an early age, her hands were always busy tending the sick, bathing fevered foreheads, bandaging festering wounds, and burying those who could not be saved during the Black Death. Death was common, hope was not.
Nevertheless, when rumours spread of a masked doctor whose methods defied all reason, Alessandra’s family sought him desperately to save her brother. Niccolo Vieri arrived, his mask glinting in the candle light, a long cane tapping against the floor as he muttered under his breath about the nature of disease. Vieri closed the door behind him, trapping his victim to his fate. What was once Alessandra’s greatest hope became their greatest fear. When she entered the room to inspect what the treatments were, the horror was revealed. Her brother lay dead, covered in glowing green flowers and spores. The doctor was nowhere to be seen, even though Alessandra couldn’t see him she swore she heard the walls whisper that the sickness was “incomplete”.
Haunted by grief and suspicion, she tracked Vieri’s experiments across the countryside, witnessing horrors that twisted her stomach: shrieking patients, grief stricken families, bloated corpses, skin crawling with green pustules. Each encounter confirmed her dread — he did not heal; he toyed, he studied, he destroyed. Determined to stop him, Alessandra enlisted soldiers to hunt the Doctor. But he fled, leaving only chaos in his wake. She was left to gather the remnants of his cruel experiments, the echoes of suffering a bitter weight on her conscience.
Weeks later, she awoke not in Firenze, but in a world drenched in fog and the scent of rot. Screams of the tormented hung in the air like smog. And there, gliding through the green haze, was him — the Plague Doctor, his mask gleaming, passing through walls, ever searching for new subjects.
Alessandra swore she would save any she could, no matter the cost. She could not stop the horror entirely, but she would bear witness, she would fight, and she would endure — even in a world where death was only the beginning.

Movement Speed (Survivor Realm): 4.4 m/s (110%)
Terror Radius (Survivor Realm): 32 meters
Power: Unwelcome Presence
The Plague Doctor exists between worlds, spreading sickness through unseen proximity.
The Plague Doctor normally walks using his cane, when in chase, undetectable, or when a survivor becomes oblivious, the Plague Doctor will unsheathe his sword.
Special Ability: Veilstep
Face a solid surface and press the Active Ability Button to activate Veilstep. The Plague Doctor will create a door into a wall and walk through it, and like a mirror appear in the same place he was standing, but in the opposite realm.
- Transition between the Survivor Realm and the Realm of the Dying
- Transition duration: 2.5 seconds
- Always exit at the same location you entered
- Ends any active chase immediately
- Cooldown: 10 seconds
Restrictions:
- Cannot be activated directly on pallets or vault locations
- Can be activated on any other solid surface
Audio Rules:
- Survivors afflicted by Oblivious or Deafened do not hear the transition
- Undetectable does not silence Veilstep audio
Realm of the Dying
While in the Realm of the Dying:
- Movement Speed: 5.0 m/s (125%)
- Terror Radius: 40 meters and can be heard by survivors in the survivor realm but the distance is indeterminable
- Survivors, auras, scratch marks/foot prints, blood pools, pallets, and vaults are invisible
- Survivor sounds are significantly louder
- Locker breathing is amplified; coughing is partially suppressed
- Pallets do not exist
Passive Ability: Sickness
Survivors within the Plague Doctor’s Terror Radius while he is in the Realm of the Dying accumulate Sickness.
- Base time to full sickness: 30 seconds
- While The Plague Doctor is within 10 meters of a survivor, progression is 50% faster
At 80% progression, survivors begin coughing.
At 100%, survivors become Sickened, suffering:
- Oblivious
- 7% Hindered
- 10% Repair Speed penalty
- Periodic coughing
- Scratch marks replaced with glowing green footprints which last 10 seconds but begin fading at 7 seconds and are only visible in the survivor realm, perks affecting scratch marks have no effect on footprints.
The Plague Doctor receives Killer Instinct for 4 seconds when a Survivor becomes fully Sickened (Visible while in the Realm of the Dying only).
Special Ability: Funeral Blooming (Realm of the Dying)
Hold the Power Button to charge Funeral Blooming. As the charge progressive, black spores arise around the Plague Doctor expanding out to 20 meters at max charge time. Upon release, all spores pulse green and dissipate.
- Charge time: 3 seconds
- Can be released early
- Movement speed while charging: 1.0 m/s
Upon release, spores pulse green in an AOE around The Plague Doctor.
If at least one footprint from the survivor realm is within range in the Realm of the Dying:
- All existing footprints for that Survivor are revealed in the Realm of the Dying
- Newly created footprints are revealed for the next 30 seconds
- Faint music plays while tracking footprints in the Realm of the Dying.
Special Attack: Funeral Bloom (Survivor Realm)
Hold the Power Button for at least 0.75 seconds to throw dark spores in a cone out to 12 meters
- 5% Hindered while charging
- After a brief delay spores pulse green and affect the survivor.
- Survivors must remain in the pulse to be affected
Against Fully Sickened Survivors:
- Cleanses Sickness
- Applies blindness for 90 seconds
- 6% Repair Speed penalty for 90 seconds
- 60 seconds of Sickness immunity
Against Non-Sickened Survivors:
- 6% Hindered for 3 seconds
- Applies blindness for 20 seconds
- Progresses Sickness up to 40% Sickness progression
Cleansing
- Cooperative cleansing as a healing action: 12 seconds (Med-Kits apply)
- If both Survivors are Sickened: 18 seconds
- Failed skill checks progress sickness in the healer
- Survivors are cleansed upon being hooked
- Sickness progression pauses on hook
- Sickness above 50% is reduced to 50% when hooked
Plague Doctor Add-Ons
Common
- Herbal Infusion – Slightly reduces the charge time of Funeral Blooming by 0.25 seconds.
- Withering Petals – Increases the duration of Blindness applied to survivors by 5 seconds.
Uncommon
- Pale Bloom – Increases Funeral Blooming range in the Survivor Realm by 0.5 meters and increases the range for detecting green footprints by 1 meter.
- Poisonous Fragrance – Increases sickness meter gain from Funeral Blooming by 5% and cap to 45%.
Rare
- Virulent Spores – Extends Blindness duration on sickened survivors by 10 seconds.
- Lingering Bloom – Green footprints last 4 seconds longer in the Realm of the Dying and 2 additional seconds in the Survivor Realm.
Very Rare
- Toxic Bouquet – Increases the Hindered effect from Funeral Blooming on non-fully-sickened survivors to 4%.
- Unstoppable Presence – Increases sickness progression rate from Unwelcome presence by 10%, and if the Plague Doctor is within 10 meters of a survivor, this bonus increases to 20%.
Ultra Rare
- Plague Vial – After hitting a fully sickened survivor, they become Exposed for 5 seconds, 10 seconds after being hit.
- Death’s Aroma – Fully charged Funeral Blooming now reduces survivor repair speed by 10% for the duration of Blindness.
Diseased Air
Healing and cleansing speed within your Terror Radius are slowed by 10 / 12 / 14%.
The effect lingers for 6 seconds after exiting Terror Radius
Restorative Torment
After hooking a Survivor, hitting an Injured Survivor within 30 / 40 / 50 seconds grants 6% Haste for 7 seconds.
Final Diagnosis
At the start of the Trial, 4 random Hooks become Scourge Hooks.
Start the trial with 1 Token. Gain 2 Tokens per Generator completed, up to a max of 2.
Each hook on a Scourge Hook consumes 1 Token.
Upon the death of the first Survivor, Final Diagnosis activates for 30 / 45 / 60 seconds.
Remaining Survivors suffer:
- Hindered (1% × Tokens Consumed) + 2%
- Blindness
- Deafened
Resolve of the Dying
After safely unhooking a Survivor, remove Exhaustion and gain immunity to negative status effects for 3 / 4 / 5 seconds.
Deep Wound is immediately mended.
Cooldown: 60 seconds.
Guardian’s Defiance
After rescuing a Survivor, the next time you are injured, the Killer is Hindered by 5% for 10 seconds.
Triggers once per rescued Survivor.
Recompense
After fully healing another Survivor without stopping, gain Endurance for 6/8/10 seconds. If the healing progress is within 80% completion and the killer walks within 10 meters the heal is immediately completed and the healer gains Endurance for 6/8/10 seconds.
