Teammate body trapped me until the killer found me and hooked me - How do i report him?

I was doing a generator when the teammate came right up and stood behind me. After I finished the generator I found that i was trapped by being body blocked.
He kept me there until I got a bunch of crows over my head and then the killer saw me and hooked me.
I have video proof and I want to report him. Its obvious to me that he's done this many times before and i want to get him banned.

Please use the in game report function to report other players then send video evidence to support - https://support.deadbydaylight.com/hc/en-us/requests/new?ticket_form_id=191426
(when uploading the video, you might find it easier to upload to YouTube, keep the video unlisted and put the link to that on the support form).
