What's one QOL adjustment you'd make to DBD?
For myself, it'd be turning down the volume on "The Entity Hungers" sound effect. For some reason, it always seems to play 30% louder than the other sounds in the game (and is the most ear-bleeding sound in the game, IMHO).

I want to be able to open DBD, and have it automatically go to the main screen. Currently I have to double click DBD, wait for the super loud chapter video to start playing, cancel out of the video, wait for the splash screen to tell me to press the space bar, press the space bar, and then wait for the main screen to load.

Hook bubble transparency. Kindred doesn’t always show the killers aura for the first second or so, which is enough for some killers to be out of the radius in time. Not only auras, but it physically blocks you from seeing killer hurtling towards you after hook.

All survivors can view their teammates perks both in the Lobby and while in game by opening the menu.

For the menus, I’d make the perk list into an actual scrollable list with filter categories and perk names and descriptions visible in the list without having to click or hover. Having a grid of often inscrutable icons was a lot more palatable when there was just a handful of characters compared to the dozens we have now. 9 pages of icons is a lot harder to search through.
For in game, I’d just have the actual names of effects (and maybe even little hints like “-33% healing speed”) next to the icons. Seeing the hex icon with no clue what hex you’re affected by, or some arbitrary add-on icon for a rarely picked killer isn’t terribly helpful.

Stop slugging to win. Stop it. Make them auto pick up if they down you or soemthing and they can't be dropped.

Allowing killers to see hook states. Now before anyone panics, I know they claim it's to stop tunneling. However, tunneling killers are going to tunnel, anyway. They obviously are keeping track of who to tunnel out. We get tunnel complaints practically every day in this community, so clearly hiding the hook states isn't accomplishing anything to stop tunneling. I'll tell you what it IS doing, though. It's preventing friendly\fair killers from NOT double-hooking someone. So many times, if I only realized that they were on deathhook, I would have gone after someone else instead. If killers saw hook states, we'd likely see people getting "tunneled out" LESS, as there is a decent amount of us killers that want a fun\fair match for everyone and would stop a final hook on someone where others have yet to be hooked.

I'd throw half the budget at fixing audio levels and making them consistent. It's incredibly frustrating to lose because you didn't hear something you should be be able to hear, or could normally hear but whoops, patch broke audio again. Headphones, amp with the gain turned up, a DAC, and the survivor is still somehow emitting zero audio 6 meters in front of me with nothing in between us.

Basic controller settings.
Deadzones, Response Curves, different sensitivity for normal gameplay vs aiming (as killer or with flashlight), different sensitivity settings for each killer. Aimdressing or whatever it’s called to be less obstructive to Controller players and lunges to not cancel themselves out. Maybe even adjust M&K to actually have limitations to turn Rate etc (iirc Oni is not supposed to be able to do 180-360 flicks though it’s very much possible on Mouse).
FoV slider for killer/first person view, with rework of Shadowborn and M&A
Phobia configs for specific phobias where it’s reasonable and applicable (eg Haemophobia and Aichmophobia aren’t really reasonable to adjust to in this very specific game - but Coulrophobia and Emetophobia can be taken care of by just changing some skins and/or sounds which wouldn’t give any unfair advantage or disadvantage)

Removal of map offerings. The games RNG is already bad enough, it’s even worse getting sent to Borgo and Garden of Misery. I say this as a Claudette main who loves using Mr Cowboys map offering and a Myers (Scratched Mirror) who uses indoor map offerings (though I let everyone go if they weren’t toxic or “in my face”).
Other than that, I’d like all serial DC’ers or first hook suiciders at 5 gens to get matched together in all of their games so the non-crybabies don’t have their game ruined anymore.

Adjustable colour scheme for auras, scratch marks, etc. I'm colourblind and on several maps I can't see scratch marks even with colourblind settings on. If I could make the Entity's presence hot pink I might have half a chance on Coldwind or Borgo.
Give ranged Killers a native crosshair, people use external ones anyway and it might make Deathslinger less painful on console.
Fix Twins already.
Survivors see what perks other Survivors are bringing in the lobby, Killer sees what items and offerings they're bringing (so I can dodge bully squads easier).
More consistent logic with hook placement, especially on multi-floor maps.
Fix Bubba already.
A clearer post-game breakdown of what earned me BP would be nice.

FOV slider for killers. Please, BHVR. It's not a big deal in terms of balancing but it would be nice, if everyone was able to play with 4 perks.
Survivors should see each other's perks. We already have it in DBD Mobile, no reason why it shouldn't be in the original game.

Some other games allow you to open overlay menu (by holding TAB). And during that overlay menu you can move your cursor and hover over icons. That would be my simple solution for this and for seeing teammates perks.

Do we actually know that Oni is not supposed to flick at 180-360°? As far as I know this is just a feature that this killer has, similar to how you can turn 180° with Billy, if your mouse sensitivity is high enough.

- Allow Survivor to see their teammates' load outs before the game
- Fix the "black bubbles" so Kindred & Bitter Murmur can work as intended
- Search bar for perks & add-ons (and make it so it works with keywords as well like "exhaustion" or "aura")
- Search bar for charms or just a better way to find them (per tome / related to character...)
- Add a "collection" tab to the store, where you can view all the outfits in a collection without having to go find each individual character featuring in it
- Option to switch lobby / main screen background

- Option to switch a killer's TR music to the default one (so I don't have to hear SM's)
- More sound options (Why is my bloodweb always so loud while the rest is fine?)

- Option to make Clown always appear as the pink elephant (or whatever non-clown related skin) on your screen
- Option to replace some Killer sounds (Plague's vomit, whatever the hell is happening during Nightfall)

That would be the logical thing to do. But the fact alone that there are no controller settings yet and higher DPI gives an advantage playing Blight should be enough to know that is not going to happen anytime soon.

One thing that I thought of when the topic of accessibility came up from the visual terror radius was that, it'd be nice if you could see the pistons of the generator moving through walls when you're within the range that you'd normally be able to hear it, on the killer side.
So, you see the aura mapwide, but when you're close enough that you should be able to hear it, you can see the pistons moving as appropriate. This covers both for those of us with poor hearing (raises hand...) and for when audio breaks and you end up not being able to hear it even though you should. Maybe a little flashing or sparking visual in time with the sound of active repair? Not sure how you'd convey that, but it'd be nice too.
