How can survivors take 3 hits instead of 2?

The title sums it up. I've just had 2 matches in a row where injured survivors were not downed after a second hit. They had no styptic agents, or syringes, or even medkits with them, no endurance statuses and no deep wounds after being hit, they did not take protection hits or no mettle of man equipped. The only thing that I saw they had in common was being close to "invitational tiles", such as blocked windows or spawned pallets. But even if I'm stunned with this "party pallet", shouldn't they go down when I hit them?
Please help me figure it out. What am I missing?

Just happened again. I got stunned with a normal pallet and hit an injured survivor and he didn't go down.

Sounds like a server thing, usually it favors the killer but sometimes you'll see yourself injuring or downing a survivor as they drop the pallet but on their screen they see themselves stunning you with the pallet and not getting hit. Used to happen a lot with Dead Hard, there's nothing you or the survivor can do about it.

Did the survivors in question have to mend?

At any point in time - nope. They were injured and I was playing dredge.

If it's the reason, then it sucks. I've had at least 2 matches with 2 survivors leaving like this in each case.

Aaaand again. This time the survivor was invitation-exposed and didn't even get injured.

It sucks but it goes both ways. I’ve seen survivors get downed or grabbed while 4 meters away. It’s hard to tell which side was actually on track.
