Poorly Designed DBD Achievements (In My Opinion)...
Hand Eye Coordination
You must play Survivor and hope that by chance your Killer just so happens to be a Vecna, this could take hours or literal days and weeks depending on your luck and is the first massive issue with this achievement.
After this, you have to hope that you roll a 20. This could never happen, thus wasting all that time queuing, or your teammates could roll 20s, locking you out of the Hand or Eye of Vecna.
Finally, once you manage to roll a 20 and get the Hand or Eye of Vecna, you have to pray that the Vecna is worse than your teammates, or that your teammates are just overall better than the Vecna.
The amount of RNG and luck needed for this achievement is beyond stupid…
To make matters worse, there is the Rightful Owner achievement, which requires the Vecna to 4K whenever someone rolls the Hand or Eye of Vecna and "retrieve the artifact", meaning that if you even manage to get the Hand/Eye of Vecna, you are likely to be tunneled out so they can "retrieve the artifact".
Hemophobia
This achievements requires you to be playing on Crotus Penn Asylum (one of two maps on the realm, meaning that Map Offerings do not have a consistent way of getting this map) and you must escape without being injured.
To put it bluntly, the only way to get this achievement is either by sheer luck, having an awful Killer, or the Killer is AFK for a massive duration of the match.
Outbreak Breakout
Other achievements for a map are often "repair the main building Generator and escape"… how about "play on this particular map that a lot of people hate, open the Exit Gates and escape TWENTY TIMES."
Both Exit Gates are opened by another play and you escape? Too bad, that wont progress the challenge since you need to be the player to open the Exit Gate(s). And the fact you have to do this 20 times to get the achievement.
Left For Dead
You must, as the final Survivor alive, repair a Generator and escape through the Exit Gates… Sounds simple right?
Repairing the final Generator is not only a waste of time since Hatch spawns in, but Hatch being closed before you finish the final Generator locks you out of this achievement.
So you have to pray that your team sucks just enough to die at 1 Generator remaining while also being good enough for that 1 Generator to have a decent amount of progress, you have to pray that the Killer does not find Hatch, and then you have to pray that the Killer does not find you or lets you go as the final Survivor.
This achievement USE to be easier to get back in the day but direct and indirect changes to how this achievement functions has made it a living nightmare to get.

I'm not sure if this still works with Left For Dead but it doesn't need to be the final gen but just a gen after everyone is dead. This might have been a bug and has since fixed but I remember seeing people mention this was how they got it. Last alive, pop any gen, killer closes hatch, escape through door. It didn't have to be the final gen, any would work as long as you finished it after everyone was dead. Here's hoping it still works that way.
I also have a couple challenges to add that I didn't think were great. You got most of the annoying ones though, I agree 100%Skilled Huntress on Console (Old)
This used to require 100 downs from 24 meters which was an insane ask. I believe it has been since been lowered to 20 but that was fairly recent; maybe this year. I got this when it was still 100 and it was a pain, even trying to farm it didn't make it any better. It's still in the top 5 rarest trophies I have.
Taking One for the Team
Requires you to take 250 protection hits from recently unhooked teammates. It's not difficult but can easily be one of the most grindy things in the game.
Edit: Honorable mention also goes to old Evil Incarnate since people could dc and ruin the entire attempt. Now with bots it's far easier; especially if you use the Meat Plant backpack strat™.

Agreed, the conditions are very tight. I haven't even started to hope to get it, it just seems too unfeasible.
Hemophobia
I actively tried to get this. I got Chapel 4 times in a row. When I finally got Asylum, I was absolutely ridiculously lucky, with the fact the Killer was almost always away from me, seemingly doing very poorly. My teammates managed to unhook each other, while I just focused gens, and just skipped right out that gate the moment I had it open. First try, but definitely pure lucky.
Outbreak Breakout
4/20 so far, despite trying my best to not get caught. I also didn't realize at first, that I personally had to open the Exit Gate for it to count, but I sure as ######### learned it the hard way.
Left For Dead
I somehow managed to get this achievement, before I paid any attention to them. I don't know. I must've just been really sweaty try-harding.
Taking One for the Team
65/250 currently. I consider it just a sort of passive progress achievement. No use in actively farming it, while there's other achievements to attempt.

The skilled hunter on ps atleast, still requires 100 downs from 24m away.

Looking into it, it seems like it's mislabeled on PS4. The PS5 version shows it as 100 while the PS4 version says it's only 20. I'm guessing the 20 is an error and it still requires 100 which is a shame. :/

I wish I could give you my cheevos. I've got this one and couldn't care less. It's baffling to me that anyone cares about achievements. They don't do anything.

No, it still requires 20 on PS4, I remember getting the achievement back when I played on Console.
The PS5 version of DBD ported over the older versions of the achievements so it's set to 100 for that reason, it's also why the other values for achievements are a bit jank.
