I came out of a game where I earned the achievement after slamming David... into a wall, not a survivor. As you can see here I got the achievement and there's bamboo behind him. Doesn't look like there's any survivor there.

Kate dropped a pallet at the same time I charged at her with Wesker's power. I got stunned and she started floating until I recharged Wesker's power, that seems to fix the bug. I assumed it was hit validation since the white lines were still around my screen.
my friend got downed near one of the exit gates and caged by the truck. The random we were with wasn't able to free him. He wouldn't die even after the end game collapse ended, all he did was wait until the killer D/C'd.
