It has two issues. It doesn't always show the explosion if you're within a certain range and it doesn't always apply Surveillance on hit if a Survivor immediately clicks back on it.1 -

Stun the Killer three times in a single match. Killers either refuse to chase me, play Killers that are difficult to stun, or my teammates burn through every goddamned pallet in under two minutes.1
