Chains Should Not Impede Progress While Actively Solving Lament Configuration
By far the only issue I have with Pinhead, albeit a relatively minor complaint, is how chains can still spawn if the player is solving the Lament Configuration. If it were not for the fact that said chains interrupted the solving process, I would not have an issue with this, but as it stands it is an annoying feature that practically punishes the player for trying to complete was is essentially a secondary objective.
Now I am not saying chains shouldn't spawn at all for the player holding the Lament Configuration, as I understand the reason why it happens in the first place, but I am saying if the player is actually attempting to perform the solve action, chains should not spawn to interrupt the process.
I propose two simple solutions:
Solution A. Chains will not spawn if actively solving the Lament Configuration. Cooldown to attempt solving the Lament Configuration increased to prevent spam/immunity (scales with how far the solving process got).
Solution B. Chains will still spawn while solving the Lament Configuration but do not interrupt the solving action.

From my experience, chains don't spawn when solving the box, but they do spawn the instant before you start solving it, and take a second or so before they hit you, so you can very easily be caught off guard.

This is how it is supposed to work!
there is a bug report on this however and I have seen it happening myself that chains can appear well after you started solving the Box.

I personally think chains should still be able to spawn while you're solving the box. You see the box's aura for the entire time it's spawned. If you neglect the box long enough for a chain hunt to start, making the box harder to solve should be part of the punishment, as it gives it more of a sense of urgency. Besides, it doesn't make it impossible to solve, just a quite bit more time consuming.

Based on RNG there could be instances where it would become impossible though, especially with the addon that increases the time needed to solve the box.

Honestly the box chains are extremely inconsistant, I held the box for ~15 seconds and only two chains spawned then I had enough time to do a whole gen before any other chains even bothered me

The problem is, that in the 3-5 times you try to solve the box, the killer can walk to you, chase you and trigger the chain hunt when downing you. Or the timing is messed up, because you couldn't solve it while he was occupied and now he came.

Chains should absolutely still spawn while trying to solve the box. They stop after a few clusters, so you can still solve it: but this mechanic is the only thing that allows Pinhead to even have a chance to stop you without a teleport.

The reason for this is to punish waiting to long to solve the box, optimally you should be solving before chain hunt begins
