Item of Obsession Achievement

So recently I'm on a big grind to get all the achievements done. One that seemed pretty interessting to me, was Item of Obsession, where you have to leave the trial while being the Obsession, unbroken and as the last Survivor.

It took me some time, but eventually I did it. I escaped, without being hit at all. I made sure I was the Obsession and I was the last Survivor. But I did not get the achievement. Since then, I have done this multiple times. Every time i made sure that if someone was dying on hook, I waited until they were dead.

Can someone tell me, if there's anything I missed?
