Loading Into a Match Is Glitching

SUMMARY OF THE ISSUE: When loading into a match, the header where the name of the map is, appears to distort. It looks like my monitor is broken but it is not. I played 2v8 and it appears to be a problem on survivor and killer where the header is broken.

PLATFORM: PS4 slim

STEP BY STEP INSTRUCTIONS ON HOW THE ISSUE OCCURED: I loaded into multiple games, survivor and killer and the header would be horribly distorted.
