That is pretty much the only good thing about the Shrine of Secrets that makes DLC not too much of a P2W concept, I do think however they should charge extra shards for the perks from DLC if the player doesn't have the DLC yet.

It is situational, if you are trying to run a vault master build (perk build where you vault really fast) it's almost necessary, otherwise, it's pretty useless.

I imagine that The Doctor would probably have medical trays around his hooks with scalpels, pliers, lethal injections, and probably some other "Instruments of Torture", as well as things that emit electricity, (circuit boards, wires, etc.). The Pig would probably have a workshop area for her traps, as well as some sort of…
