  • As others have already said devs confirmed multiple times it's just a pure gameplay decision, but yeah, lore-wise it can be explained quite accurately by the Entity's powers to bend timelines and dimentions at it's will, and so making multiple "Dwights"or "Claudettes" met in the same Trail.

  • Pretty sure they get re-created every time a new Trial starts. That would also explain the different locations of gens, pallets, chests ecc... every time a new Trial begins, even if it's the same exact map. Not sure if devs said something about this tho.

  • If phones were to be found inside the Entity's Realm they would just be things created by the Entity itself like the Medkit, the Flashlights ecc... and even if it was a real phone, it would certently do not work as it would in the real world. In my opinion, Ghostface is simply saving the photo for himself as a reminder.
