Is Pyramid Head's (and I guess terror radius in general) bugged?

I just played a game where I got a 4k as nurse, moried 2, hooked 1, and the other bled out. Had only her base perks. Didn't get the achievement. I know it says merciless victory but I thought it only required you to get a 4k with the killer's orig perks now, am I wrong?
